This program is specifically available to those who are Medicaid-eligible and based on a medical necessity determination.   • Medication Administration

  • Hygiene Care (Bathing, Dressing, Grooming)

  • Feeding Assistance (Including G-Tube Care and Tube Feeding)

  • Incontinence Care

  • Transfer Assistance

  • Wheelchair Assistance

  • Vital Signs Monitoring

  • Daily Living Activity Assistance

  • Turn or Reposition Assistance

  • Trach Care (Including changing and cleaning)

  • Respiratory Treatment (Including Suctioning, Bagging via Trach or Mouth)

  • Observation and Communication (Reporting issues to nurse or doctor)

  • Changing Sheets and Bed Linens

  • Collecting Treatment Plan Information

Eligibility Made Simple: GAPP Program

To qualify for GAPP services, your child must:

  • Be under the age of 21.

  • Need assistance with daily living activities such as feeding, bathing, grooming, or mobility.

  • Be enrolled in Medicaid.

If your child meets these requirements, they may qualify for free in-home care services. The program is not a waiver service, meaning there is no waiting list — your child can begin receiving care immediately after approval.
