Our Process
Consultation and Care Plan Development: We begin with a consultation to understand each patient’s unique needs and create a tailored care plan.
Parent Involvement and Program Approval: Parents actively participate in the development of their child's service plan, with final approval taking up to 30 days.
Caregiver Matching: We match the right caregiver to each patient, ensuring compatibility with both care requirements and personality.
Medicaid and GAPP Program Enrollment: We are an approved Medicaid provider through GAPP, offering in-home care for children under 21 based on medical necessity.

Skilled Nursing Services: Medication Administration, G-tube Care and Cleaning, Trach Care, Respiratory Treatment (Nebulizer, Vest, and CPT Treatments), Observation, Emergency Safety Protocol
Personal Care Services: Hygiene Care (Bathing, Oral Care, etc.), Feeding Assistance (Including G-tube Feedings), Dressing Assistance, Turn or Reposition Assistance, Transfers, Wheelchair Assistance, Assistance with Therapy Exercises at Home (e.g., ROM Exercises, Gait Trainer, Stander, etc.)
